We are here with Alessandro and his ICLs have finally arrived, two magnificent toric V5 model ICLs of the latest generation to correct his vision defect. Alessandro does not have a very high vision defect, we have around four diopters in one eye and three in the other, therefore an astigmatic, myopic vision defect and we have chosen to implant the phakic lens because Alessandro has a cornea that is not really perfect, he has oblique axis astigmatism, we are a little uncertain about the future evolution of this cornea.
We have been following him for six years, we do serial maps every year, and corneal maps and his cornea is absolutely stable. Another characteristic of Alessandro is that he has a stable vision defect, he is very young but his vision defect has been stable for six years. He has expressed his desire to have refractive surgery because he is uncomfortable wearing glasses, even though he sees well, but he loses them, breaks them, when he rides a motorbike they fog up, when he goes skiing they fog up, so he has all a series of impediments.
Tomorrow we will perform bilateral surgery, which we don’t usually do, but he is in a very important and busy study period. We chose a quick surgery, with a sequential implant in the two eyes. We are going to implant this lens which has this cookie shape, we will make a small cut in the temporal part, where there is white, on the limit between white and the color of the eye, we will insert the rolled lens and inside the eye the lens will open, after which we insert it behind the iris.
The lens is very small, floating inside this little demo pen. The surgery takes place locally and recovery will be very fast. Alessandro, do you want to tell us something about what you expect tomorrow?
Are you afraid, are you worried?
No, I am not worried, because anyway I have been informed for many years about this surgery, so I have no particular concerns about this problem.
Have you read anything about this lens? You know that many patients tell me it’s a foreign body inside the eye and are afraid to put it in. How do you feel about inserting a foreign body?
No, honestly I’m not afraid of that, and I know that it can be removed at any time.
