Congenital hypertrophy of the retinal pigment epithelium (CHRPE) is an asymptomatic congenital lesion of the ocular fundus, classified into typical forms (solitary and grouped) and atypical forms. The atypical variant is associated with familial adenomatous polyposis (FAP).
Atypical CHRPE lesions, linked to FAP, present with hypertrophy and hyperplasia of the retinal pigment epithelium, retinal invasion, and retinal vascular changes.
Atypical CHRPE lesions are commonly smaller (50-100 µm) than solitary lesions, may present with depigmented lacunae and halos, and are associated with a significant percentage of bilateral lesions.
CHRPE is often identified incidentally during routine ophthalmological examinations, and the presence of multiple or bilateral lesions may suggest an underlying FAP.
